Management Information SySTEM
The Lazarus Foundation, Inc. receives funding from U.S. Department of Housing and Urban Development and/or the State of Wisconsin Department of Commerce to provide services for homeless and near homeless individuals and their families. A requirement of this funding is that this agency participates in the Wisconsin Homeless Management Information System. This system collects basic information about clients receiving services from this agency. This requirement was enacted in order to get a more accurate count of individuals and families who are homeless and to identify the need for different services.
We only collect and share only the information that we consider to be appropriate. The collection and use of all personal information is guided by strict standards of confidentiality. A copy of our Privacy Notice describing our privacy practice is available to all consumers upon request.
Our residents’ information will be shared with other area agencies that participate in the network unless the resident requests that his or her information remain confidential and be only available to case workers and intake workers within this agency. Please talk to a case manager or intake worker about keeping your records closed if you are interested.
